Endobariatrics

What exactly is an endobariatric procedure? This sounds confusing at first mention. However, the answer is quite straightforward and is progressive. This procedure has transformed minimally invasive surgery into, well…. to be precise, no surgery! No incisions! No scars!

This procedure is completely endoscopic. This is not surgery. Rather, the stomach volume is reduced. The reduction is both axial and radial, or simply stated, north and south, and east and west, respectively.

The stomach is sutured with an endoscopic device. The scope is placed safely down the esophagus into the stomach.

The DEVICE is a Boston Scientific product that provides suturing capability.

The PROCEDURE is called an endoscopic sleeve gastrectomy (ESG) and is performed on the inside of the stomach, called intraluminal.

This was approved by the FDA–Food and Drug Administration. The results are strong and can be compared to a formal, stapled sleeve gastrectomy without any scars. Patients can experience approximately 18-20% of total body weight loss or 40-60% of excess weight.  However, patients undergoing a laparoscopic sleeve (LSG) will typically lose 50-70% of their excess weight.

This is gaining more than just interest and attraction, but instead making headline news. The Boston Scientific device has undergone modifications and improvements.  The device can be controlled by one operator, has a short learning curve, and the company provides excellent support staff and information. Patients experience quick satiety and tolerate smaller intake after an endoscopic sleeve.

Surgeons and gastroenterologists can perform the procedure; however, you may need to contact a surgeon if you experience a complication.

Complications include, but are not limited to:

  • Bleeding
  • Infection
  • Perforation
  • Sepsis
  • Peritonitis
  • Stricture
  • Nausea/vomiting
  • GERD, reflux
  • Pneumonia
  • Spasms
  • Failure

The procedure is considered safe and typically has a low complication rate. Recovery is very quick, even in comparison to the sleeve gastrectomy.  Patients are intubated and have general anesthesia provided by an anesthesiologist. The procedure will generally last one and a half hours.

Most ESG patients return to work the same week as the procedure. Patients must continue making healthy choices and eat a balanced diet, including micro and macronutrients. However, they can start soft solid foods in 1-2 weeks. The results are slower but usually well controlled. The health benefits include weight loss and weight-related diseases or comorbidities, including heart disease, diabetes, high blood pressure, and high cholesterol.  Patients need to follow the recommendations and adhere to the plan.
